Signed by the contributor separately and only where her story is to be used in advertising for CLIRIUM® ME programmes or Victory Naydenova’s books. It is not a condition of publication in the magazine.

Consent to Advertising Use of Name, Image and Story

I, ______________________________ (full name), consent to Victory Naydenova and CLIRIUM® ME using my name, image and excerpts of my story published in Clirium’s Magazine (Issue No. ___, “____________”) in advertising for CLIRIUM® ME programmes and Victory Naydenova’s books (Fla. Stat. § 540.08; Art. 152.1 of the Civil Code of the Russian Federation) on the following terms:

1. Only excerpts of the text I approved may be used, without changing their meaning; any new wording requires my written approval.

2. Every use carries the disclosure: “[Name] has taken part in CLIRIUM® ME programmes. This story reflects personal experience; results vary.” (16 CFR Part 255).

3. No claims of treatment, cure, diagnosis or guaranteed results may be made.

4. Advertising aimed at consumers in Russia complies with Law No. 38-FZ, including labelling.

5. Compensation: ☐ none / ☐ as follows: ________ (if compensation is paid, this is disclosed in the advertising).

6. Term: ___ months from signature; I may withdraw consent by writing to support@cliriumsmagazine.com, after which new use stops within 10 business days.

7. Governing law: the laws of the State of Florida; as a consumer I retain the protection of the mandatory laws of my country of residence.

8. This consent may be signed electronically (15 U.S.C. § 7001 et seq.; Fla. Stat. § 668.50).
